Massage TherapyStatement of Policies
New Piricing Effective January 1st, 2009: Fee: There is a fee of $60 for each one-hour session; $85 for each one-and-one-half-hour appointment, $50 for a 45-minute session, and $35 for each one-half hour session. Different prices apply to specialty massages. Fees may be raised on a yearly basis, but will be comparable to rates in the Capital Region. A MasterCard, Visa or gift certificate number is necessary to secure an appointment but not required to pay for it. If a client does not have a MasterCard, Visa or gift certificate, he or she must prepay by check before an appointment will be scheduled. Clients may also secure appointments by gift certificate number. We will accept cash, check, Sunnyview gift certificates, MasterCard, or Visa for payment. We do not accept Domani gift certificates at Sunnyview. Payment is due at the time of the appointment. We do not participate in insurance reimbursement. Gift certificates are transferable, but may not be redeemed for cash. Expiration dates on gift certificates are firm. Contraindicated Conditions: There are some medical conditions for which bodywork techniques should be modified, or are not appropriate. We ask you to give as complete a medical history as possible to help us give you the best session we can. In some cases, we may not be able to receive you without a release from your primary healthcare provider. In trying to reach your primary healthcare provider, it may be necessary to reschedule the appointment. If you have a serious medical condition, you may want to clear massage through your physician prior to scheduling an appointment. Statement of Intoxicants: We will not work with any client if we suspect the use of intoxicants or recreational drugs of any kind. The therapist reserves the right to terminate the session at any time if he/she feels that client behavior is inappropriate whether or not intoxicants are suspected. The client will still be responsible for the fee. Client Cancellations: We ask that the client be respectful of our dependence on their presence at the scheduled appointment. Please only book appointments when you are sure you will be able to keep them, since there is a high demand for appointments. If unforeseen circumstances require you to cancel an appointment, please give us 24 hours notice, or have a friend keep the appointment for you. When appointments are canceled within 24 hours or missed, it is our policy to charge full payment for the session. Unfortunately, we cannot be responsible for forecasted weather conditions or client illness. Client Lateness: We will hold your session for up to 15 minutes after the start time of your appointment. We will extend the 15 minutes if you call to let us know how late you will be arriving (this will not apply to 30-minute appointments). If you arrive late, the session provided will be the best we can give you in the time remaining. The fee remains the same even though the appointment will be cut short by the amount of time you are late. If We Change or Cancel a Session: Due to booking errors or emergencies, we may need to change or cancel a session, but we will make every attempt to call you with as much notice as possible before the session. It is therefore imperative that we have daytime and evening phone contacts. In the event that we cannot contact you before you arrive for your appointment, and we cannot find a replacement therapist to do the session, we will offer you a gift certificate. If we can provide a replacement therapist, or do not have proper contact information, we will not offer the gift certificate. Nature of service being rendered: We provide massage therapy and bodywork including general Swedish, sports, deep tissue, therapeutic, pregnancy, and stone spa massage. Individual therapists may have training in other bodywork modalities. Completion of training certificates may be required, in which case therapists may include only what they are trained to perform. Massage therapy is not a curative form of therapy. We do not diagnose, prescribe exercise or cure illness or dysfunction. Massage therapy is for general relaxation and assisting in the healing process. Massage therapy is not to take the place of medical management of illness, disease or dysfunction. Client Recourse: If the client is unhappy or dissatisfied, he/she may speak with the director of administrative patient care services. If the matter is not handled to the client’s satisfaction, the client may contact the New York State Office of Professional Discipline. Both clients and therapists have a right to stop a session if either the client or therapist feels it is right to do so. We reserve the right to cancel or stop a session if alcohol or recreational drug use is evident, if we feel that the client’s behavior is inappropriate, or if, in our judgment, the session poses a threat to the health and safety of the client or therapist. |
