Sunnyview offers both in- and outpatient therapy to people who have suffered from heart problems. Every year, the Inpatient Cardiac Rehabilitation Program helps people with heart disease return to productive, active lifestyles following an illness, injury or surgery. In 2000, Sunnyview provided therapy to a patient following his heart transplant surgery. The Cardiac Program Sunnyview's cardiac rehabilitation team provides comprehensive medical management and intensive rehabilitation for patients and their family members. Combining all aspects of therapy-physical, occupational, dietary, speech and language, and psychological/social-the Sunnyview team uses an integrated approach to suit the individual needs of each patient. A typical inpatient stay is about 10 days. |

The
Rehabilitation Team
Sunnyview's cardiac rehabilitation team consists of board-certified cardiologists, physician assistants, cardiac nurses, physical and occupational therapists, dietitians, social workers, psychologists and respiratory therapists. Every patient has
unique needs and recovers differently from the health problems that can
be associated with cardiac rehabilitation. The rehabilitation team works
with you and your family to develop goals and a personalized program.
The team will help you increase your strength and endurance, monitor and
prevent complications related to cardiac surgery or heart disease, and
educate you and your family about heart disease and care. |

| A
graduated exercise program, carefully coordinated and supervised with state-of-the-art
telemetry monitoring of your heart functions, will lead to maximized activity
and independence. You will learn to do more with less effort, increase your
strengths and relearn how to perform the activities of daily living (such
as climbing stairs, doing housework, walking, etc.) safely and independently. Your will also learn more about how your heart works, how to participate in safe exercise, how to manage your medicines, how to maximize your nutritional needs, how to manage stress, and how to conserve energy. You will also learn how to modify risk factors in your own lifestyle to minimize the recurrence of heart disease. |

Visitors Your family is invited to attend daily educational sessions and may also attend some of your therapy sessions. Education seminars take place each afternoon from 3:30-4:30 pm. The support of family
and friends is important to your recovery. However, equally important
is your rest and therapy. Generally, visiting hours are Monday through
Friday, 4:30-8 pm and on weekends from 11 am to 8 pm, following therapy
sessions. If visitors come during your therapy hours, they are welcome
to observe the therapy process. What to Bring You will need personal hygiene items, night wear, a robe and slippers. Most of the day, you will be wearing casual, comfortable clothes and footwear that is suitable for walking and exercise. For your convenience, a washer and dryer are available on the inpatient unit. Discharge The Sunnyview cardiac
rehabilitation team develops an individualized discharge plan for each
patient based on his or her unique needs. All patients are referred back
to their own primary care physician and cardiologist for follow-up treatment.
The Sunnyview team makes therapeutic and medical follow-up recommendations
and coordinates these with appropriate community service agencies to ensure
a smooth transition to the home setting. Sunnyview's staff will also make
a follow-up appointment with the referring physician within two weeks
of discharge.
